Online Liability Waiver Instructions:
Click on the 'ONLINE' button above and you will be taken to Docusign, a secure website that handles our liability waiver form signatures. Alternatively, you can choose the 'PRINTABLE' version, complete it by-hand and mail it to PSBO. Follow the instructions below to complete the online version.
Clicking the ONLINE button above will take you to Docusign's information page. Enter your name and email address - this helps us keep track of who has signed the form. The page will look like the image to the right. Once complete, click the 'Begin Signing' button.
The next screen will display the liability waiver details as shown in the image to the right. Click the 'Continue' button at the top of the page to review the liability details.
The following screen provides the same liability waiver details but with the sections needing a signature marked with small yellow boxes. Click each of the boxes and if your name does not automatically appear, type in your name. Once complete, click the 'Finish' button. A pop-up window will indicate that you have signed the document correctly. There is an option to download a copy of the waiver in that same pop-up window should you wish to keep a copy for your records.
The Photo Release is not required, but please, consider allowing us to use the photos we might take of you at a training session or in the field. Please also feel free to send us some of your photos of you and your team while volunteering on our projects. We love to share the awesome work you are doing in the field!
